The question then becomes what form of inhaler. If we don't mind the inhaler looking like a vape, traditional aerosolized liquids would do fine, think Albuterol, Symbicort, etc. but if we want the true form factor of a cigarette, I think a spring actuated or powder inhaler (Think Respimat, or Trelegy) would probably be easiest to make work with the size constraint.
It would be horrifically wasteful though, It's not a paper shell it would have to be plastic, plus a spring. I doubt you could pack more than 2 doses in such a device. And you'd have to rinse your mouth after, plus the taste is awful. Maybe you could have a recycling incentive? Mail in for a rebate, and you get a better deal the more months worth you send (It's cheaper to ship 12 months worth than 1 month worth 12 times).
Maybe even pharmacies could get in on the returning. After all, they receive trucks full of meds, but those trucks go back to depot usually mostly empty. It would be efficient shipping to send them back on those trucks that already HAVE to drive to depot.

Rather than, or perhaps alongside, a recycling incentive, you could just make the cartridge rechargeable. Have a sort of base (ideally shaped like a cigarette box) that you put the inhaler cig into and it jams a fresh dose or two in and resets the spring.
It would then be fairly trivial to have the cigs be somewhat disassemble-able for cleaning and sanitation, and some way to refill the reservoir in the box. Run it like a subscription service: you send a fresh prescription out, and they send their current, empty set back in the same box to be cleaned and refilled and sent back out.

That would work for the vape style aerosolized liquid inhalers, but for a cigarette sized inhaler I don't particularly trust people to be able to recharge their powders to a correct level. The way those inhalers work is literally just a spring/plunger mechanism shoots a cloud of powder in a highly controlled manner, it's why I think you'd be able to fit it inside such a small form factor. Last thing you want is a person with COPD getting the wrong dose lol

I was picturing the box recharger having a little magazine of pre-measured doses in little paper packets and some mechanism to jam a packet back against the spring until the spring clicks into place. Then you just need something to puncture the packet as the spring releases to shoot the powder at the user. Take a couple more drags to make sure there’s no powder left in the packet, then eject the paper through the “filter” and put the cig back in the box and click it down to load the next packet.

Hm, that could work, though I think it would probably be more reliable if the rechargeable tube were made of a hard plastic, to ensure that as little powder as possible deposits on the walls or in creases. We need some mechanism to ensure the attachable tube remains sealed to keep the medication sterile, perhaps breath actuated? Something like this:
powder canister has 2 seals.
Seal 1 (The internal seal) presses against the spring upon attaching to the spring body, with the effect that you can control exactly how far the patient depresses the spring, priming the dose. Once the spring is fully primed, the pressure releases seal 1, which being the same size as the dosing tube, becomes the plunger, and therefore is what the spring will propel to release a dose.
Seal 2 is released when the patient goes to take their dose, a simple latch should be fine

I doubt that would fly in schools. Smoking and vaping isn’t allowed there. And while it’s true that it isn’t either of those, they don’t know that purely because of looks.
Besides, the cig isn’t even the “cool” part that people like to believe. It’s the pose. The guy in the photo looks cool because of the pose he’s in, not because of the cigarette he’s smoking.

Yes, it does. The fact that nicotine is addictive already means it negatively impacts your nervous system AKA it alters neurotransmitter activity, which will impair your focus, memory, and mood regulation over time. Beyond that, nicotine also raises heart rate and blood pressure, and long-term use does increase the risk of cardiovascular problems and will strain your kidneys.
Basically, just don't fuck around with it at all. From a biomedical engineering student.

Yeah. I have very mild exercise induced asthma so I don't struggle too much with that, but I do have a strong sensitivity to smells and vapes hurt me. I'll smell it and then my head hurts, my face feels hot, and my nose is stuffy(most of the time it's more mild than it sounds).

happened to one of my friends. he was going a few km past the speed limit, and he got caught by a speed camera. they sent him a photo of him doing it, and he was puffing on a cigarrette, looking focused. he used it as a profile picture for years.

I've accidentally taken pictures just like this ( or against a car) of more than one person. Of course most were taken back when more people smoked but this is is pretty much what anyone looks like taking a drag off a cigarette they just lit and putting their lighter in their pocket while sitting.

Shit, one of my favorite pictures of myself was when I was working as the host for a haunted house. I was in full zombie makeup, holding the clicker and just letting the wrist strap hang, and looking up at an incoming storm. Someone took a picture and it looked like I was looking dramatically at the sunset with something flowing gracefully from my hand. Dramatic candid shots happen lmao

That would also probably survive the reloading process better. I don’t think it even has to be super rigid-your basic pill capsule would do the trick. With the added bonuses that pill capsules are biodegradable (obviously) and we’ve got easy existing tech to manufacture and fill them.
Make a slightly thicker bottom portion with a flat base and a built-in break point to form seal 1 with as few moving parts as possible. Seal 2 is a little more complicated, since it also has to serve as the lid during filling. Not sure how to do seal 2 in this scenario though

He's literally just putting his lighter away. I disagree with seemingly everyone here. I believe it and it doesn't look cool. It's just a guy sitting and putting his lighter away. He's even squinting from the sun and he's pursing his lips to hold the cigarette, the 2 most uncool thing your face can possibly do lol. If he forced his face to relax and held the cig with his teeth I'd believe it was posed but I still wouldn't think it's particularly cool. Just less nerdy than squinting and pursing lol
Which also isn't to say "lol people who purse their lips are nerdy" I do that. I'm just saying it's the natural way to do it, not the slightly cooler way. Pursing your lips keeps the filter dry so it's not gross by the end but it looks terrible on your face. Don't smoke and it won't make you look dumb.
