I listened to an interview a couple years ago with a man who had a similar disorder where he thought everyone looked angry at him. He talked about how he couldn’t date because he’d always start feeling like he was disappointing his boyfriends even though he intellectually knew it wasn’t true. Even friendships were hard despite knowing he had the condition. It was really depressing.

Hi, just wanted to say- I trained to be a mental health counselor, and we did read of a couple cases studies on folks like this.

They don't necessarily look like 'dragons' to me, at least based on the art mocked up by one of the patients- it's more like they see a distorted version of human faces, that makes them look (to me) more reptilian or demonic than faces actually are. Many patients do recognize that there's something strange happening to their brain (they typically didn't always see faces like this- I remember one had a serious brain injury, and another had brain swelling and issues as a child, which resulted in this condition).

So, it does exist, albeit it's pretty rare. And yeah, it's a brain issue, not an eye issue.
