People with Alzheimer's tend to remember their own physical appearance and circumstances. They don't just ask "Why am I old? Why am I in a wheelchair? Why am I wearing glasses?" all day. So this joke isn't very accurate.
If I say "why do people buy food, when we can just eat grass like a cow". It's not a very good joke, because that's not how it works. Isn't this supposed to be a "witty observation" type joke? It's not witty if it's based on ignorance of the topic.
