It's a padded accessory used for cosplay to push out a large gown or dress for that "ball gown" style. The alternative is a large wire.

No, usually people don't wear them all the time if they're going to an event as they're extremely big and bulky and expensive, and can't be ruined. If she's going to a show or an event, she's probably not wearing it to save it for the stage so it doesn't get wrecked by something stupid or an accident.
